The National Health Service (NHS) in the United Kingdom is set to pilot a groundbreaking artificial intelligence (AI) system aimed at predicting disease risks and premature mortality in patients. This innovative AI system, known as AI-ECG risk estimation or Aire, utilises data from electrocardiograms (ECG) - a common and cost-effective cardiac test that measures the electrical activity of the heart. Scheduled to launch trials in mid-next year, the project will begin at Imperial College Healthcare NHS Trust and Chelsea and Westminster Hospital NHS Foundation Trust, with more locations expected to be announced.

Aire is designed to enhance the detection of cardiac structural issues that are not easily visible to medical professionals. Dr Arunashis Sau, a clinical research fellow at Imperial College London's National Heart and Lung Institute and a cardiology registrar at the Imperial College Healthcare NHS Trust, elaborated that Aire is intended to supplement, not replace, clinicians. "The goal here is to try and use the ECG as a way to identify people that are at higher risk, who will then maybe benefit from other tests," emphasised Dr Sau.

The benefits of early identification through Aire are profound. By interpreting ECG data, Aire aims to detect conditions such as diminished heart pumping function, indicative of heart failure, enabling the early initiation of beneficial treatments. Dr Sau highlighted the potential to reduce hospital admissions and enhance life expectancy by starting interventions at an earlier stage.

Dr Fu Siong Ng, a reader in cardiac electrophysiology and consultant cardiologist at Imperial College Healthcare NHS Trust, specified that multiple studies would commence in hospitals next year to assess the accuracy of AI models using ECG data for diagnoses. The initial phase will involve several hundred patients, with plans to expand substantially in later stages. Dr Ng outlined a vision where, eventually, every ECG conducted within the NHS could be processed through Aire to provide clinicians with comprehensive diagnostic predictions and health risk assessments.

Research showcased in Lancet Digital Health highlights Aire's potential: it successfully identified a patient’s risk of death within the subsequent 10 years in 78% of cases. It also performed well in predicting future heart failure, serious heart rhythm problems, and atherosclerotic cardiovascular disease. Dr Sau asserted the importance of trust between patients, doctors, and AI, underscoring the AI's ability to discern complex cardiac structural and functional information that may elude human clinicians.

This initiative is part of a broader trend of integrating AI into the UK healthcare system, fundamentally altering traditional medical practices. The advent of AI-powered technology is propelling healthcare towards increased precision, efficiency, and personalization.
